You should make a mix of both by analysing each of your main topics and then judging by the amount of monthly views (use google adsense keyword tool for that) focus on 10 keywords first then 20 and so on until you have struck a happy balance and place well.

When you have a very long list of keywords go through with one and delete those that do not describe your product. You may need to do this several times until your list is down about 30 or less right keywords for your product.

What exactly do you wish to promote? Is this mostly geared to the general field of health or do your have a specific focus or niche benefit? Keywords go with the task at hand and the most successful campaigns are centered around a specific goal. Your keywords should be tailored to the exact benefit or service you choose to promote. Without knowing much as about what you're promoting or your goals I would generally recommend starting with less competitive long tail keywords. As you build a reputation you'll eventually rank for the more competitive terms.

It would be a good idea to start with less competitive keywords, as these will be able to draw in traffic. Once you have established your site, then you can start targeting more competitive ones.

Well, I think you should focus on low competitive keywords first. Once you've done that, opt for competitive ones.
