Contacting Google for Reinclusion Request

I have had my websites removed from Gooogle.

How do I go about contacting Google to ask if my sites are blacklisted or what is happening.

Any help appreciated

http://www.google.com/support/bin/request.py (It seems to be down at the moment)

Make sure to put "reinclusion Request" in subject line.
