I'm not sure also if we can ask google about that. Imagine if there are 1 million new adsense account that will ask to review their ads placement, I think google adsense team can't handle that. That's why they provide some ads sample and tips on where to put and what type of ads that you are going to use.

Not sure about this. First you have to read policy of the Google. Then ask.

No you can not

If you're not modifying the code, or trapping the clicks or changing the way ads are processed, it should be all right to implement a click-counter script like the ones suggested.

Do be careful not to change the ad code without realizing it, though.

If you´re asking from an advertiser point of view, you can block certain industries and even sites where your adsense ads would be placed

Google already give the guidelines on placing ads on websites. They even give tutorials and tips on placing ads that they proved to be effective. Check out this guidelines and look for tutorial videos.

I think that all that you can is place ticket in their system and wait what they reply to you.

I think read first the terms and conditions, and follow the guidelines.
